The enclosures and/or attachments (may/do) contain identifiable and protected health information (PHI) in accordance with the Health Insurance Portability and Accountability Act of 1996 (HIPAA). Pursuant to our obligations as a Business Associate under HIPAA, we are making this transmittal with the understanding that you will use this PHI solely for the purposes related to the legal representation of our client and that it will be maintained in a confidential manner. We further request that you: 1) Make no additional disclosure of the PHI without our express written permission; 2) Notify us immediately if you become aware of any unintended disclosure of this PHI; and, 3) Upon your completion of the authorized use of this PHI, return it to us, or destroy it in a way that makes it no longer identifiable (i.e. shredding). By your use of these documents, it is our understanding that you have agreed to comply with these requests. If you cannot agree to comply with these requests, we ask that you return these documents without accessing the enclosed PHI.
